This form is used to suspend your membership while unable to attend your club. As per your membership conditions please be aware of the following conditions: - If paying via direct debit your payments will cease for the period you specify. Please be aware if a payment is due in the next 48 hours we may not be able to stop that payment, however, your membership payments will cease for the amount of time you specify. - If paying upfront this time will be added to the end of your membership (maximum time added is 3 months) - There is a once off $5 fee per suspension - Maximum suspension time is 3 months, minimum is 1 week - All suspensions must have an end date; you can not suspend until further notice - I understand that my payments will re-start without notice at the end of my suspension period that i have specified.
This form is used to cancel your membership at Healthworks. Please ensure you are aware of the following membership conditions when cancelling. - There is a 14 day cancellation period. Please provide as much notice as possible - If you cancel your membership and rejoin at a later date, we can not gaurentee any previous special rates and you may need to pay another Joining fee upon renewing. Alternatively you can suspend your membership. - You can still train in your club up until your membership end date. (This will be specified in your confirmation email) - For upfront memberships, we are sorry, there will be no refunds after your 48hr cooling off period. If your circumstances change you have the option of transferring your membership to a friend or family member, or another Healthworks club. To do this there is a $50 transfer fee. - If cancelling within 48 hr cooling off period please contact the club directly.
