This form is used to suspend your membership while unable to attend your club. As per your membership conditions please be aware of the following conditions:

- If paying via direct debit your payments will cease for the period you specify. Please be aware if a payment is due in the next 48 hours we may not be able to stop that payment, however, your membership payments will cease for the amount of time you specify.

- If paying upfront this time will be added to the end of your membership (maximum time added is 3 months)

- There is a once off $5 fee per suspension

- Maximum suspension time is 3 months, minimum is 1 week

- All suspensions must have an end date; you can not suspend until further notice

- I understand that my payments will re-start without notice at the end of my suspension period that i have specified.

If you are suspending more than 3 months you will receive a confirmation email within 48 hours of submitting your suspension, if you do not please contact your club. If you do not have an email address we can fax you a copy or please ask staff for a copy of the suspension note.
